Patient Information Package
Please print the following forms, fill them out and bring them with you for your first appointment.
In addition to routine annual Physicals we do the following three physicals:
School/College Sports
Info: Many colleges require or strongly prefer that all new students undergo routine physical exams prior to attending classes.
DOT Physicals
Info: The Department of Transportation (DOT) physical exam helps certify that a driver does not have any physical, mental or organic condition that might affect his or her ability to safely operate a commercial motor vehicle. The exam incorporates the required medical assessment (including physical exam, and review of medical history) and includes the DOT medical card.
Sports/Camp Physicals
Info: The number of deaths in recent years of athletes has pushed many schools to require annual pre-participation sports physicals. A pre-participation exam is also endorsed by the National Federation of State High School Associations. A history (a discussion of your child's symptoms and overall health) and physical exam will help to target children who may be at risk of health problems while playing sports.
