Billing Information
Three Lower Counties Community Services, Inc (TLCCS) will bill all the insurance companies you provide at registration. Once payment is received from the insurance company, TLCCS,Inc will bill you for any remaining balance. It is important for you to understand the terms of your insurance coverage. If the plan requirements are not followed, you could be held financially responsible for all or part of the services received in TLCCS. If you have any questions regarding your bill, please call your patient account representative at 410-651-3354.You may use your MasterCard, Visa, Discover, or American Express card to pay your portion of the Bill. It is our goal to assist all of our patients to find payment options as appropriate. Please be aware that we reserve the right to collect unpaid bills.
If You Do Not Have Insurance - Our patient service representatives (PSRs) and case workers will assist you in applying for Primary Adult Program (PAC), Medical Assistance or Sliding Fee.
Sliding Fee Program - Federal Poverty Guideline will be used for the Sliding Fee Program. If your income falls within the guidelines, we encourage you to apply. You will receive a letter and a card valid for a year from the Sliding Fee Coordinator within 10 days of the approval of your application which includes proof of income documents.

Payment Policy
Payment is expected at the time of your visit. This includes co-payments required by your insurance company. We offer a Sliding Fee Program to ensure that our services are available to everyone, regardless of ability to pay. Please inquire about the Sliding Fee Program or payment arrangements if you need more information.
We accept cash, personal check, credit and debit cards from VISA and MasterCard, as well as payment from all insurance companies. A fee may be charged for any checks returned for non-sufficient funds.
Non-payment
It is our goal to assist all of our patients to find payment options as appropriate. Please be aware, however, that we reserve the right to collect unpaid bills.

Insurance
We accept most major insurance companies and managed care health plans, including Aetna, Care First Blue Cross Blue Shield, Tricare, MDIPA, Cigna, as well as Medicare and Medicaid. Your insurance card contains the correct information we need to bill your insurance for you—please bring your card to each visit.
We participate with most managed care plans. We recommend that you check with your managed care plan to ensure that the health center is named as a participating provider in order to obtain your best reimbursement. If your plan requires you to choose a primary care provider (PCP), be sure to name your health center provider.

Sliding Fee Program
Through funding from the U.S. Bureau of Primary Health Care, we offer a Sliding Fee Program. This program is based on income and family size and reduces the amount you pay for health center services. If you qualify, you may pay only 20 – 80% of the cost for most services. You may be eligible for this program even if you have insurance. Applications are available in the waiting and exam rooms at each clinic. All applications are confidential. Our case workers will help you fill the forms.
Reduced Fee Form: Federal Poverty Guideline will be used for the Sliding Fee Program. If your income falls within the guidelines, we encourage you to apply. A Slide Card will be issued and needs to be presented at the time of your visit.

Proof of income is required to process your application. A copy of a current Federal Income Tax Return is strongly encouraged as proof of income. However, documents listed below are also acceptable:

You will receive a letter and a card valid for a year from the Sliding Fee Coordinator within 10 days of our receipt of your completed Sliding Fee application. A completed application includes your proof of income documents.

TLCCS is an FQHC: A federally qualified health center (FQHC) is a type of provider defined by the Medicare and Medicaid statutes. FQHCs include all organizations receiving grants under Section 330 of the Public Health Service Act, certain tribal organizations, and FQHC Look-Alikes. Requirements for Indian Health Service funded FQHCs may differ from the requirements for FQHCs receiving Section 330 grants and for FQHC Look-Alikes.
There are many benefits of being an FQHC. For FQHCs that are PHS 330 grant recipients, the biggest benefit is the grant funding. For new starts, funding up to $650,000 can be requested. Other benefits include:
- Enhanced Medicare and Medicaid reimbursement
- Medical malpractice coverage through the Federal Tort Claims Act
- Eligibility to purchase prescription and non-prescription medications for outpatients at reduced cost through the 340B Drug Pricing Program
- Access to National Health Service Corps
- Access to the Vaccine for Children Program
- Eligibility for various other federal grants and programs
